In-Office Eyology Evaluations
The Expanded / 60-minute Evaluation*
This is a 60-minute evaluation.
This evaluation is for individuals with moderate complaints/symptoms and who are already taking medications. In this evaluation we will address complaints and symptoms and focus on helping you regain balance and a better quality of life.
Nutritional and other recommendations are given.
*This evaluation includes 1 followup/update for $60 that must be used within 6 months of first evaluation.
The Comprehensive / 60-75 minute Evaluation*
This is a 60-75 minute evaluation.
This evaluation is for individuals with a specific diagnosis (or multiple), major complaints/symptoms and multiple medications.
Nutritional and other recommendations are given.
1-FREE followup within 6 months of first evaluation
*Please know that Eyology appointments are booked back to back therefore it is vital that you are early or punctual.
Tardiness will cut into the time of YOUR appointment and could result in cancellation.
In the case of no-shows and late cancellations there will be no refund for appointment.
We also ask that you refrain from smoking or applying colognes/perfumes before your appointment!
If you wear colored contacts they will need to be removed before your appointment.
As of July 1st, 2016 all appointments must be prepaid (cancellation 24 business hours or more in advance to your appointment time will guarantee full refund)
This is a 30-minute evaluation.
This evaluation is intended for individuals who have already been in and want to make sure nothing new has come up, discuss symptoms that haven't improved or to have regimens tweaked.
